Pine Park Health

In-home primary care for senior living residents

Pine Park Health brings value-based primary care to people who live in senior living communities across Arizona, California and Nevada. Clinicians treat residents in their own apartments, handling prevention, screenings, chronic disease management, lab work and diagnostic tests, and teams return to each community weekly, working alongside facility staff. Same-day and next-day appointments help residents stay out of emergency rooms and hospitals. The care model is designed to work for patients, families, community staff, providers and payers at once, and more than 185 communities now partner with the practice as it expands quickly.
