Value-based care for primary care practices

It creates value-based contracts across a wide range of health plans, which inverts the traditional fee-for-service model: instead of paying physicians for the volume of appointments and procedures, the arrangement pays them for keeping patients healthy. That strengthens continuity of care and aligns the incentives of practices, health plans and patients around the same outcome.
